Cereals containing gluten

Coeliac disease (autoimmune), wheat allergy (IgE) and non-coeliac wheat sensitivity — three different conditions under one entry.

Anexo II, entrada1: “Cereals containing gluten, namely wheat (such as spelt and khorasan wheat), rye, barley, oats or their hybridised strains, and products thereof”

This one entry covers conditions that have nothing in common except the grain. Coeliac disease is an autoimmune reaction to gluten that damages the small-intestinal lining and is diagnosed by serology and biopsy, on a gluten-containing diet — which is why you must not remove gluten before being tested. Wheat allergy is an IgE food allergy that can cause immediate hives, vomiting or anaphylaxis. Non-coeliac wheat sensitivity is a symptom-defined diagnosis of exclusion. The label rule is the same for all three; the medicine is not.

Who gets it, and does it last

Coeliac disease is lifelong and the treatment is a strict gluten-free diet under dietetic supervision, with follow-up. Childhood wheat allergy is often outgrown. A distinct and easily missed form is wheat-dependent exercise-induced anaphylaxis, in which wheat is tolerated normally but causes a systemic reaction when eaten before exercise; its marker is omega-5 gliadin.

Where it hides

This is the hardest allergen to live with in this region, because rye bread is the staple of the Estonian, Latvian, Lithuanian and Finnish table and barley runs through the traditional cooking of all of them. Oats are grown and eaten heavily here too, which makes the certified-oats question a practical daily one rather than an academic one.

Cross-reactions

Wheat, rye and barley proteins overlap enough that all three are excluded in coeliac disease. Oats are the argument: most people with coeliac disease tolerate pure oats, but ordinary oats are milled and transported with wheat, so only oats certified as gluten-free are used. Grass pollen allergy is not wheat allergy, despite the botanical relationship.

The labelling detail

The specific grain must be named in the ingredient list and emphasised there — bold, capitals, a different colour or underlining. Separately, "gluten-free" is a regulated claim: Commission Implementing Regulation (EU) No 828/2014 sets it at no more than 20 mg of gluten per kilogram, with "very low gluten" at no more than 100 mg/kg for specially processed cereal products.

Exempt from declaration under Annex II:

Precautionary wording — “may contain”, “made in a factory that also handles” — is a separate matter: it is voluntary, unstandardised and carries no threshold. How strictly to treat it is a clinical decision, not a reading of the label. The labelling explainer sets out the whole frame, including what happens in restaurants and bakeries where there is no pack at all.

Qué se lee mal

Going gluten-free before a coeliac test is the single most common and most expensive mistake: it can make the diagnosis impossible without a supervised gluten challenge months later. "Wheat-free" is also not "gluten-free" — spelt is wheat, and rye and barley are neither.

Preguntas frecuentes

El Reglamento lo formula así: Cereals containing gluten, namely wheat (such as spelt and khorasan wheat), rye, barley, oats or their hybridised strains, and products thereof.

Las fuentes inesperadas rutinarias son soy sauce, which is usually brewed with wheat; stock cubes, gravy granules and sauce thickeners; sausages, meatballs and pâtés using rusk or breadcrumb; beer and malt vinegar; communion wafers; some sweets, liquorice and ice-cream cones; barley malt extract in breakfast cereals.

Sí, 4: Wheat-based glucose syrups including dextrose; Wheat-based maltodextrins; Glucose syrups based on barley; Cereals used for making alcoholic distillates, including ethyl alcohol of agricultural origin.
